Client Background
A general hospital in Yemen with 100+ beds and multiple departments: Emergency, Surgery, Internal Medicine, Pediatrics, and OB/GYN. Staff of 200+ including doctors, nurses, and administrators. Entirely paper-based medical and administrative records system hindering care quality and increasing costs.
Problem & Context
The hospital relied on paper records and disconnected legacy systems, causing: (1) duplicate patient data entry at every visit wasting staff and patient time, (2) difficulty tracking patient medical history and accessing past lab results, (3) medication dispensing errors due to lack of pharmacy integration and drug interaction checking, (4) billing and insurance claim delays taking days to process, (5) inability to analyze performance KPIs like average wait times and bed occupancy rates, (6) lack of integration between clinical and administrative departments.
Our Solution
We built an integrated HL7/FHIR-compliant system covering: (1) patient management with comprehensive EHR including medical history, tests, lab results, radiology images, and medications, (2) smart appointment scheduling with wait time reduction and patient reminders, (3) pharmacy with comprehensive drug database, expiry tracking, drug interaction alerts, and e-prescription integration, (4) laboratory with results automatically linked to patient files and critical result alerts, (5) radiology with DICOM/PACS image integration, (6) billing and insurance claims with automatic pricing based on insurance policies — cutting claim processing by 70%, (7) HR management for doctors and nurses with shift scheduling.
Key Features
EHR (HL7/FHIR, 100+ beds, 200+ staff)
Unified digital patient file via HL7/FHIR protocols containing complete medical history, diagnoses, lab results, radiology (PACS), reports, and medications. Serves 100+ beds and 200+ staff across 5 departments.
Pharmacy-Prescription Integration
Comprehensive drug database with automatic drug interaction and dosage verification, expiry tracking, and e-prescriptions directly linked to patient records.
Lab & Radiology (PACS Integration)
Lab results and DICOM radiology images (PACS) automatically linked to patient files with critical value alerts for urgent findings. Supports automated equipment result import.
Billing & Insurance Claims
Instant billing and insurance claim generation with automatic pricing per patient policy and insurance provider. Reduced claim processing time by 70%.
Architecture
Multi-tier web architecture with Next.js frontend, Node.js backend, and PostgreSQL database. Runs on the hospital LAN with remote access capabilities for administrative staff. Uses HL7 and FHIR standards for health data exchange with external systems and laboratories. Supports integration with lab and radiology equipment through standard interfaces.
Security & Performance
Compliant with the highest healthcare information security standards: patient data encryption at rest and in transit, granular role-based access by medical role (doctor, nurse, pharmacist, admin) with every access logged, complete audit trail for all read and write operations with timestamps, encrypted daily and weekly backups, health data privacy compliance, and medical network isolation from the public internet.
Our Approach
We executed a department-by-department rollout strategy to avoid disrupting critical healthcare services. Starting with the Emergency Department and EHR (highest priority), then adding Pharmacy and Laboratory in phase two, followed by Radiology and Billing. Each phase took one month with intensive training and a pilot run before transitioning to the next phase.

Results Delivered
“The hospital transformed completely from the paper age to full digitization. Now all patient files are available electronically, and the pharmacy and lab are directly connected to the patient record. The difference in care quality is enormous.”
